§ 4-33. Records.

  • (a)

    Each ambulance provider shall complete a report on each call for service to which they are dispatched, in the form of the state's PreMIS or other form designated by the county.

    (b)

    Each ambulance provider shall report to the county on a monthly basis concerning activity during the preceding month. This report shall be a copy of the PreMIS or other form designated by the county for each call to which the provider was dispatched. This form shall be used as the basis for billing patients not exempt under section 4-34(d).
