§ 4-28. Application for ambulance.

Application for a franchise to operate ambulances in the county hereunder shall be made upon such forms as may be prepared or prescribed by the director of emergency services and shall contain:

(1)

The name and address of the applicant and of the owner of the ambulance.

(2)

The trade name, if any, under which the applicant does business or proposes to do business, along with a certified copy of an assumed name certificate stating such name or articles of incorporation stating such name.

(3)

A resume of the training and experience of the applicant and of each person who will operate the vehicle and attend to patients.

(4)

A description and copy of state permit for each ambulance owned and operated by the applicant.

(5)

The location and description of the place or places from which it is intended to operate; and the ambulance response district it is intended to cover.

(6)

A description of the schedule (i.e., hours and days of the week) for which the applicant proposes to provide ambulance service, and any special conditions under which the applicant intends to operate (i.e., provision of back-up ambulance service only, provision of non-emergency transportation service only, etc.).
